She brought a baby with her who was still a few months old. the baby was under pediatric for brain malformations but stable. she was carrying the baby on straps. they referred the mother for depression. she introduced herself started telling her stories. She is a single mother, her ex husband left her after they got married. her family also abandoned her for marrying him. She now lives in rented home, in which she was unable to pay for the past few months. She did admitted of past mistakes in which we did not delve into too much during our first interview. Last week I took a history from middle aged female patient complaining of loss of urine output (anuria). From the general look she was highly obese. She was very helpful in cooperating with us taking the details and measuring the blood pressure. She even called her son on the phone to assist me on the history taking. As I continued the sheet, her past history indicated she had multiple medical problems. She was positive with heart conditions and diabetes. As our Arabic vocabulary are quite limited, we didn't managed to understand everything she said, hence to came out with a provisional diagnosis. But we had some suggestions from reviewing the history we took, her complaint may be due to her uncontrolled diabetes. One even suspected lupus nephritis! Then a moment later the doctor came. And suddenly everything changed. We were, and I, was wrong from the beginning. We failed to understand her real complaint, which was a vaginal discharge. It was caused by urinary tract infection...

Our Respiratory department is 2 weeks long, 8 days to be exact. What did I learn? 1. Read from various sources of medical books, (like me I read Oxford clinical handbook) but never leave the department books. The questions will always be asked from the department books. 2. Do not skip classes, especially the clinical sessions. They are upmost important to develop your clinical skills. 3. Do not smoke, and avoid passive smoking. 4. Increase your lung capacity by exercise holding your breath everytime you are entering/exiting the department because the of it's foul-smelling-smell caused by anaerobic organism infection I guess. 6. Enter OSCE exams early, the questions are usually simple, short and sweet based on my one time experience and what Nizam, a widely renown pokemon trainer (biiznillah) told me. Today we had class at Nephroplogy Unit. Dr Muhammad Ali explained and asked us questions on acute kidney injury (AKI) and chronic kidney disease (CKD). We were asked quite a lot of questions. Questions on basic medical knowledge, but somehow we were unprepared. I can see the expression of mild disappointment from Dr Ali. But he kept his cool and continue explaining as we, mostly did not respond to his questions. By the end of the class, one of my friend said to me, "5 years (in medical school) and we knew nothing." I would disclaim that we were unprepared because we didn't know which topic we will be learning every day for the elective posting. But that's not an excuse. So, what are the lessons learnt today? 1. Short notes on today's lesson: CKD differs in duration than AKI, which is >3 months. CKD relies on the level of GFR. Hemodialysis is done for stage IV & V. Untuk mengetahui dengan lebih mendalam kita akan membuat klasifikasi punca-puncanya kepada dua; yang pertama ialah muntah yang berlaku secara fisiologikal 1 , dan kedua secara patologikal 2 . Muntah fisiologikal Muntah fisiologikal atau mabuk hamil ini berlaku semasa awal waktu pagi dalam bentuk mual atau muntah dan berlaku pada trimester yang pertama (3 bulan pertama kehamilan). Before every summer break, elective posting or attachment in hospitals are one of the common topics being discussed among medical students especially for those in clinical years. There will be many things lingering in our mind when we talk about it. How to apply? Is it tough? What should I equip myself with? Can I cope? There are fears, and there are expectations. But like what Naik always said, just do it! Who can apply? Any medical students who are: 1. Studying in institutions listed on Jadual Kedua Akta Perubatan 1971 . Refer Page 43-60, 2. Local IPTA or IPTS, 3. Still studying. Postgraduates cannot apply except registered under overseas Majlis Perubatan, applying under seksyen 6 Akta Perubatan 1971 .  You guys are encouraged to apply to do elective posting in government hospital, be it clinical or preclinical students. Asas kepada dakwah kita adalah mengajak manusia kepada keselamatan di dunia dan akhirat. Jika wasilah kepada dakwah ini adalah sebagai seorang doktor, maka dia bukan sahaja perlu menyelamatkan nyawa pesakitnya, tetapi juga iman dan ruhnya. Tanggungjawab kita amat besar, perlu kuasai kedua-dua bidang ilmu. Oleh kerana ramai yang tidak mempersiapkan diri sebaiknya semasa usia siswa, mereka gugur sewaktu memasuki fasa pekerjaan. Ada yang meninggalkan terus bidang perubatan, adapun yang lain tidak lagi bersama dakwah. Mereka tidak tsabat dan berupaya untuk menangkis halangan-halangan yang menyerang. Adapun virus dan halangan itu ialah: 1. Ada manusia yang diuji dengan sifat gopoh kerana 'nafasnya pendek' serta kurang cekal.  Akhirnya mereka tidak sampai kepada matlamat. (al-Ahqaf:35) 2. بسم الله الرحمن الرحيم Tadi baru habis menelaah embriologi.Banyak betul perubahan-perubahan dan nama struktur-struktur yang perlu dihafal, seriously .Pada mulanya memang berniat nak baca balik lecture terakhir berkenaan folding of the embry o sahaja ,diulangi, 'SAHAJA' ,tapi sebab proses ni pun adalah kesinambungan daripada peringkat-peringkat penciptaan bayi bermula dari Fertilization [1] ,Cleavage [2], Implatation [3] , 2nd Week Development [4] ,3rd Week Development [5] , Embryonic Period [6], kemudian diselangi beberapa medical term seperti Placenta Previa [7] , Decidua [8] ,Chorion [9] dan Chorionic Villi [10]  yang belum dikuasai sepenuhnya,maka nak tak nak kenalah revise semula-__-' -memang tak elok study last minute,seriously- Dan  buat sesiapa yang faham,pasti akan diakui bahawa proses embriologi adalah proses yang amat spesifik dan tidak mungkin ada saintis dan doktor yang mampu melakukan sesuatu yang setanding dengannya.Ana rasa Keith L.Moore [11] dah fah...
