One day in Nephrology Unit

Today we had class at Nephroplogy Unit. Dr Muhammad Ali explained and asked us questions on acute kidney injury (AKI) and chronic kidney disease (CKD).

We were asked quite a lot of questions. Questions on basic medical knowledge, but somehow we were unprepared. I can see the expression of mild disappointment from Dr Ali. But he kept his cool and continue explaining as we, mostly did not respond to his questions.

By the end of the class, one of my friend said to me,
"5 years (in medical school) and we knew nothing."

I would disclaim that we were unprepared because we didn't know which topic we will be learning every day for the elective posting. But that's not an excuse.

So, what are the lessons learnt today?

1. Short notes on today's lesson:

CKD differs in duration than AKI, which is >3 months. CKD relies on the level of GFR. Hemodialysis is done for stage IV & V. What if the patient has stage V CKD and asymptomatic, give supportive treatment.

For AKI, we depend on the level of serum creatinine (>0.3mg/dL/48hrs) and oliguria (<40ml br="" day="" hour="" kg="" ml="" or="">
Indications for dialysis:

Absolute:
-Pulmonary edema, hyperkalemia and metabolic acidosis.
Relative:
-Encephalopathy, vomiting, peripheral neuropathy.

The concept of dialysis. IV inlet and outlet are both venous, blood circulated by pump. Method of filtration and diffusion are applied.

2. Toughen basic knowledge. Keep repeating them. These are not new knowledge, we just forget them. I thought, "Nasiblah dr Ali ni sabar, agak-agak masa HO nanti takkan nak blur memanjang".

3. Attend classes. Not only for our exam requirement, or our vow/ amanah with our parents, scholars etc, but the process of learning learning in presence of your professors has many benefits eg. allows interactions, lecturers usually emphasize points and important concepts etc.
